IMPORTANT: Please make sure you are submitting this return, AFTER you have reviewed our return requests page. Improperly submitted return requests are voided and need to be resubmitted again according to the instructions given before we can process them.

IMPORTANT NOTE 2: Do NOT use the address on our contact page for product returns. This is a SEPARATE office. The CORRECT address will be sent via the RMA confirmation email that will be sent after your request has been accepted.

An automated email will be sent to confirm that your request was received. Please allow us 48 hours of the next business day to review your request and for one of our customer service representatives to contact you to complete the return transaction.

Your return request is considered official as of the date that you submitted it using the form below, regardless of the processing time it takes to confirm your request.

This form can be used for:

• Hair Analysis Kit refunds. (366 days from purchase, not submitted to lab)

• Health Program Consultation refunds. (366 days from purchase, health consultation appointment was cancelled or declined)

• Health Supplement refunds from any supplement purchased DIRECTLY from our website. (90 days from purchase, unopened without a broken seal. A 15% restocking fee is applied, initial shipping charges are not refunded)
